Inspire Center for Counseling provides the following information in accordance with Texas Health and Safety Code §181.105.
Requesting Your Health Care Records
You have the right to request access to or a copy of your health care records maintained by Inspire Center for Counseling.
How to submit a request
Submit your request in writing through our secure Client Portal. After signing in, use the secure messaging option to tell your therapist that you are submitting a “Health Care Records Request.”
If you do not have access to the Client Portal or cannot use secure messaging, call Inspire Center for Counseling at (832) 757-5014. We will provide a secure method for submitting your written request.
To help us identify the correct records, please include:
Your full legal name and date of birth;
Your current telephone number;
The name of your therapist;
The specific records or information you are requesting;
The approximate dates of service;
Whether you want to review the records or receive a copy;
Your preferred secure delivery method; and
Your signature and the date of your request.
If you want records sent to another person or organization, such as another health care provider, attorney, school, or insurance company, we may require a signed authorization identifying the recipient and the records you authorize us to release.
Inspire Center for Counseling may verify your identity and authority to access the requested records. For requests involving a minor or another person for whom you are the legal representative, documentation of your authority may be required.
Please do not send private health information, records requests, or copies of identification through the general website contact form or ordinary email. Contact us by telephone if you need assistance accessing the secure Client Portal.
Requests will be reviewed and processed within the time required by applicable state and federal law. We will notify you in advance of any legally permitted copying or delivery fee. If access to any portion of the requested records is restricted or denied as permitted by law, we will provide the information required by applicable law concerning that decision.

The counselors at Inspire Center for Counseling are regulated by the Texas Behavioral Health Executive Council through the Texas State Board of Examiners of Professional Counselors.

Filing a Consumer Complaint With the Texas Attorney General
A consumer may file a complaint with the Consumer Protection Division of the Texas Office of the Attorney General.
To file a complaint:
Visit the Attorney General’s File a Consumer Complaint page.
Review the available complaint categories and select the form that best describes your concern. The Privacy Complaint form may be appropriate if the concern involves the collection, use, disclosure, or handling of personal or health information.
Have the following information available:
The name and contact information of the business or individual;
A description of the concern and relevant dates;
Information about any steps already taken to address the concern; and
Any supporting documents you choose to provide.
Complete the complaint form in one session because the Attorney General’s online form may not save an unfinished complaint.
Complaints and attachments submitted to the Texas Attorney General may be subject to public disclosure. Do not include Social Security numbers, financial account numbers, copies of identification, dates of birth, private health information, or other unnecessary confidential information in a complaint.
